We evaluate the safety and effectiveness of the use of implanted DBS electrodes to create a therapeutic lesion before removal. Methods: Revision or removal of DBS electrodes was required in two patients who had previously been implanted by DBS. We performed a series of in vitro experiments to confirm that DBS electrodes could be used to generate radiofrequency lesions and to evaluate the relationship between the radiofrequency parameters and the size of the lesion. With this information, and with the approval of the hospital ethics review board, implanted electrodes were used to create incremental radiofrequency lesions in the talamo in one patient and in the sub-talmic core in another. The procedures were performed under local anesthesia with contiguous DBS lead contacts connected to the active and reference sites of the RF generator to create a bipolar lesion. Results: A 51-year-old man with essential tremor and a thalamic DBS requires repeated battery changes secondary to the demands of tolerance and high tension. Instead of replacing the battery, a radiofrequency thalamotomy was performed using the existing left DBS electrode. In the 6-month follow-up examination, the successful injury provided almost complete tremor control. A second patient, a 50-year-old man with Parkinson's disease who had undergone deep bilateral sub-almic brain stimulation, developed skin erosion over DBS hardware. A sub-talamic core lesion was performed through the right DBS electrode. The position and size of the lesion were confirmed with magnetic resonance. Conclusion: The lesions can be done through chronically implanted DBS electrodes in a safe and qualified way and can produce therapeutic benefits. Parkinson's diseaseParkinsonIdiopathy with Parkinson's disease: Deep brain stimulation and injury When we refer to 'surgery' in Parkinson's disease, it usually refers to deep brain stimulation (DBS). Deep brain stimulation involves placing a wire with electrodes at its tip in one of the three target sites in the brain - the talamo, the pallidus globus or the subtalmic core. A few days after connecting to an implantable pulse generator (IPG). This small unit is inserted under the skin on the chest wall. When IPG turns on the electric pulses stimulate the target area and cause a change in Parkinsonian symptoms. Before introducing DBS in the 1980s, the only available operation for Parkinson's disease meant selectively damaging certain cells in the thalamus and gobus pallidus called 'lesioning'. Although this sometimes relieved the symptoms the damage was irreversible and the operation was risky. With DBS you can check the insertion of the electrodes to confirm that they have been properly sidated and there are more opportunities to 'fine the adjustment' through the IPG. The technical advances that have made this operation possible have been the development of computed tomography and MRI that allow the surgeon to locate the target site with great precision. Although it is not essential, electrodes are usually inserted with the awake patient so they can provide evidence that the target has been reached. For several of the people interviewed who had not had the operation, the possibility of being awake while their brain was being manipulated was enough reason for them to dismiss the very idea of submitting it.
